How to apply for Florentia Village membership?

Do I have to fill in all the information?
Fields marked * are required items, including name, card number, ID card number, birthday, gender, address, phone number and Email. It is critical to provide complete information to receive full member benefits.
Because membership benefits include birthday rights, if you want to experience or change the birthday, you need to show your ID to Florentia Village Customer Service customer to update this information.
What are the current member's benefits?

How to replace a card if my membership card is lost or degaussed?
Advocate environmental protection and energy saving, all villages has been fully upgraded to the electronic membership card, your electronic membership card once activated immediately effective.
If the physical card is damaged or lost, please visit the Customer Service Center to provide your personal information and have a new e-card issued.
Once your new e-card is issued, the old card will not be valid anymore and all points will be transferred to the new e-card. The validity period will remain the same as the old card.
If your card is lost or stolen, you must report it to the Customer Service Center. Your new e-card will be issued once your personal information is confirmed.
If you need to change your personal information, please visit Florentia Village's WeChat official service account to change the data on your personal e-card or go to the Customer Service Center for your physical card.
How to apply for membership card upgrade?
The Florentia Village has five membership cards, which are White card,blue card, silver card,gold card and Platinum card. <br/>Register to become White Card, Blue card can be applied for when one day's purchases reaches RMB 700(or HKD 700); it can be upgraded to a silver card if the purchases reach RMB 10,000(or HKD 10,000)within two months; silver cards can be upgraded to gold cards if the purchases reach RMB50,000 (or HKD 50,000) within five months. gold cards can be upgraded to Platinum card if the purchases reach RMB100,000 (or HKD 100,000) within one year.<br/>Membership card upgrades can be applied for at the service center.
